If a piece of equipment is malfunctioning or a working condition is unsafe, let the union know right away. Choose the company, include the equipment details, describe the problem, and attach photos or a video if you can.

Tell us the type — stack, truck, van, RTG, forklift, or other — and its number or ID.

Explain what's wrong and attach photos or a video so the issue is clear.
